There isn't really the technology for even making a bot which can moderate images. It's much cheaper hiring actual people than spending billions developing an AI which can do as much as even moderate basic images.

There are ways to design shirts in which a way that it seems alright content however if placed with the corresponding shirt it could create mature content. Not to mention this doesn't make much sense as you can just make the shirt however you please in the first place(Therefore you could just add the details to the already made shirt, if that makes any sense)

Adding details like, hair extensions, gun shot wounds, etc. would be useable and useable with a variety of different shirts. And you're right regarding your first comment, but that doesn't change because there are two layers. That is an issue with just one.
