Perhaps since most people wouldn't know history from around the world, they could have a setting to enter what continent they reside in. And then receive questions from countries in that continent or more specifically, their country. This would solve the "global questions" issue.

Suggestion: To prevent the same questions from being asked twice in the same round, put the used questions in a queue. When a new question goes into the queue, (and the queue is at max capacity; maybe 6 questions) one comes out and can be used again. Same goes for categories so that they're not used twice in a row.
