Report: He was defective yet helpful to the social intake, you could obviously see he was confused and misunderstood. He had some internal bleeding you could see metal pecks of dustus from him banging his head on the cell. He was going mad towards the end and was quick to judge, though he trusted me he was an unsuccessful at showing his emotions to me. He was obsessed with mac and cheese and why he was hungry so he is progressionate to his emotions. I believe this concludes my report,

Report: Subject seems fine at first answers the first few questions, he thens start to have a bit of amnesia
as he seems to have been in a SCP test but he doesn't remember what. He says that he was with someone with a red tie.
He seems a bit calm but worried, and doesn't rememebr what the SCP did to him. He was in a test with mutiple people.
The subject seems to have no injuries or worries, but he seems to still have amnesia, Maybe he doesn't remember his disabilties.
He did not do the eye test well he had low vision, I ask him one more question about the msot detaiedl thing he knows in the past 30 minutes.
He answers in a not detaield way and forgets most of it.

Report: At first the subject seemed scared almost as if he was going to die, I try to keep him calm but he still feels unsettling.
He answers the first questions fine a bit worried at first. A SD then walks in and disturbes him.
He begins to answer every question in one sentence. He has been in no test recently. The subject begins to show symtpons, he is sweating.
He begins to feel disoriented and odd. He backs away from me. He has terrible eye sight and begins to now know much. |

In the beginning of the medical test I asked the Class D if he had encountered any SCPs recently, He described encountering a white like SCP, When I clarified SCP 096 it turns out that was the SCP he had encountered. He felt alot of fear when encountering SCP 096. After this I asked the Class D if he had felt depression and or [Blocked Word] recently, He replied with "Not all of the time" after this I concluded the medical test.
My notes:
I have learned from this test alot on how Class Ds think and how they feel when they are exposed to an SCP. This information will be helpful in the future. |

At the start the Class D was easy to communicate with, He was very co operative with the doctors. I asked the Class D how he was feeling this day, He replied with "good". After this I asked him if he had encountered any SCP recently, He described encountering a tall, white creature. When I clarified SCP 096 he realised this was what he encountered, He described feeling alot of fear when encountering SCP 096.
After this I asked him if he had felt any depression and or [Blocked Roblox Word] recently. He replied with "not all the time". After this I concluded the test.
My Notes: This test helped me learn a bit more about how Class-D think in the presence of SCPs. This helped me alot and I hope to learn more in the future. |

Events: At the start I asked him a few questions, These include: How are you feeling today (Good), Which division do you wish to excel in (Medical Department), Have you ever encountered any SCPs (No). At this point I concluded the test. At the end I gave the personnel a Lollipop.
Notes: The Personnel ZeroFlaws is doing well mentally and is good to go. |

Subject says he has been feeling a little queasy. Subject has not recently come into contact with any SCP. Subject has a headache and complains of hearing sounds of buzzing flies or elephants and some times.
Subject read down to the green line indicating a 20/20 sight. However got no letters correct after the green line.
Subject complains of how often he is left in his cell to "rot". Subject appears to be depressed and mentally unstable.
Check-Up results: Failed. He was VERY nervous and was extremely uncooperative. Do not use on tests. |
