When I used DevEx, I just sent the request to cash out, an extra tab in "My Transactions" was created called "Developer Exchange" and under that it'll show the date, ROBLOX, Developer Exchange, and the amount; "12/10/14 Roblox Developer Exchange (Completed) 500,000" Though it should say Pending instead of Completed. Eventually once they get the request and read it, if they feel you're all good to use DevEx, they should send you an email which provides further info about it. Hope this helps. :) -- CodeWrighter

Yes, I did have to fill out and send in a form, but that was after they sent me an email in response to my request to cash out. So all that is needed to cash out is clicking the cash out button then waiting for them to accept or decline it. If they accept it, then they'll send you an email like I said. :) -- CodeWrighter
