I like Boo (SMK/MK64/MK:SC/MKDS/MK8DX), because you will be invisible from other players, drive off-road and also steal an item. In Mario Kart: Super Circuit, it will slow down the leader. Makes this item useful.
But one thing I hate about this item, if the computer player uses it, it will affect to the player. Doesn't matter if you're a leader or not, if you were near to the computer player with using Boo or how much powerful your item is. Talking to Mario Kart: Super Circuit and Mario Kart DS! But still best item.
I don't like Lightning (SMK/MK64/MK:SC/MK:DD!!/MKDS/MKWii/MK7/MK8/MK8DX), because you will shrink down, slow down, lose your items and worst part about this... Fall down when you're flying! And you can be also squished by a normal-sized player. This basically ruins my fun. |

Best: Red Shell - Homing projectiles are always useful especially if the target has nothing to protect them. Even better if you get 3 of them.
Worst: Lightning Cloud - This item actually can work against you. No wonder that it was only in Wii. |

In my opinion...
Best Mario Kart item: The Super Horn. It's rare, but in situations where you are about to meet the Blue Shell, it can really save you and is a reason why I like this item.
Worst Mario Kart item: The Blue Shell. I hated those homing missiles. They home on the racer who is in first place and HITS them, especially when they're close to the finish line on the last lap.

Best: Lucky 7 from Mario Kart 7. Gives you basically one of each usable item.
Worst: Blooper. When you are playing by yourself, this thing does nothing since the opponents are computer. Multiplayer though they are alright.

Best: Golden Mushroom because you can spam that sucker to victory

I'm surprised no one has talked about the Bullet Bill. I know people will say it's because it's only obtainable in the last spots, but it's basically an auto-pilot for noobs. But in my opinion, the best item in Mario Kart has to be the Golden Mushroom. It's basically a multi boost that lasts for a limited amount of time and it's GREAT if you want to move up some spots.
The worst? Well it's definitely not the Blue Shell. I am honestly sick of this item getting crap for just being a "homing missile that doesn't even help your position". THE WORST Mario Kart item is the Coin. It is THE MOST USELESS item in the game and serves no purpose but to troll you. |

Worst: Boo since it's useless for action replay on the DS while using 300cc and it slows you down

Okay, sounds fair.
But the Blue Shell is still in the lower half of the items but not rock bottom IMO. Because it has it's redeeming points as it could make a match completely unpredictable and in rare circumstances help you out. I say rare circumstances because it could happen. |

Best: Boo, Triple Red Shells, Green Shell, Lightning
The Boo basically gives you free invincibility, and a free item. In fact, using the Boo makes offroad driving treat your kart like it's on the road, negating any offroad speed penalties. Granted, the Star boosts your speed and treats your kart as though it's always on the road, and knocks racers around, but doesn't get you a stolen item.
(Basically same reasons as the OP.)
Triple Red Shells do well to bully your way to 1st or some high position if you're getting knocked around-though it is annoying to lose a shell to a stray item.
1 G. Shell is better than 3 if racing, as it's far more reliable against any incoming red shells. Battle? Then it's triple green shells.
Plus, firing it right means the shell can bounce off a wall and hit an opponent on the sides.
Lightning's been a loose cannon for me. This has successfully saved a race going poorly for me once, but at the same time I agree with the OP that it's awkward if someone nails you while you're mid air.
Worst: Blue Shell(kind of.)
To be fair, I regard worst as loose cannons, so the Lightning should be here. Blue Shells are by the Lightning, as they're amazing to knock back the person in 2nd if you time reversing correctly, but if you're far ahead it can ruin your lead. |

Best item: Bullet Bill.
Worst Item: Throw skills out of the windows and use blue shell, the most notorious item of all. In MK Wii, it is at least possible to avoid them with a well timed drift. In MK8, it's a lot difficult, you need the horn. Good luck getting that in first place. |

Nobody else bringing up how completely useless the Blooper is? Especially in MKDS and 7 where you could just use the lower screen.

Worst- PIRAHNA PLANT. HOLY CRAP this thing is so annoying in Mario Kart 8. It's so freaking common, and if you come within 20 miles of somebody you might as well have gotten hit with a blue shell. It sucks. |
