Purpose: What information are you trying to get from this experiment?
Planned Procedure: A few sentences on how you plan for your test to go. This is to help tests be more organized.
Hypothesis: What do you think the result of your experiment will be? It's alright to be wrong, this just helps keep the test going in a specific direction.
Procedure: Step by Step what actually occurred. This should be close to the planned procedure, but things can change once a test starts.
Data: What were the actual results of the experiment?
Casualties: Number of personnel that died on the test, including CDs, Combatives and FP. List the number of casualties and the division of each one. Remember, the test does not end until all personnel have left the containment area.
Conclusion: What does your data mean? How does it affect the SCP and the Foundation? What further testing should be carried out to further the research? Answer those three questions and say anything else you think is important.
